This way, if there were ever a question as to the legitimacy of the document I can refer to my journal and know for a fact who the witness was. I have used a relative as a witness, I have never been told otherwise (this is only on witnesses for loan docs, NOT credible witnesses for notarial acts). I hope this helps.

I can't officially answer all your questions, but I will tell you what I do. I DO know that you should print the witnesses name underneath their signature.
