Obviously, from a science point of view, having one's bpdy suddenly increasing in height, weight, musclemass and so on would be unimaginably painful (Warren Ellis made this point about Banner's transformation in 'Ultimate Human' a while back). Comics are speculative fantasy though, and to try and go against that too much kind of takes the fun out of it.
I think that when either Bruce or Jen are undergoing the change they are not just increasing in size but aquiring a superhuman ability whose sole purpose is to allow them to survive and adapt to the physical stresses of what is happening to them. Both Hulk and She Hulk are able to heal from injury and pain at an abnormal rate so there's no reason that this ability would not start to kick in when their other powers do.
