Here's another option..in the arc w/ Abadox ..recall he claimed to have s helmet he found from the cancerverse. It was a prime helmet and implied to be Rich's. I think Abadox even promised it to Sam. It seemed pretty important and got a semi splash page.
When everything went down in that arc..somehow the helmet got lost in the shuffle and was never discussed or shown again. It really killed me as some weak sauce writing.
