I had a standpipe like the one you mentioned and the way it worked was the water went in the large pipe and the exhaust went into another pipe that went up nearly to the top of the big pipe. The water only went to the pipe exiting at the side of the standpipe and the exhaust went up above that so that the exhaust pushed the water out. This arrangement did two things, didn't let the water back down the exhaust and into the engine and the water cooled the exhaust as it went up the inside of the standpipe and then as it exited down the port side to the stern. This arrangement is all right if you don't get a hole in the exhaust pipe that lets the water back into the engine. As far as the smell in the head is concerned I'de say there was a leak in the system. This leak could be in the engine compartment or the head. Check the engine compartment first and then take the wood cover off the standpipe and inspect it. If you don't find it in these two places it might be in the exhaust hose that runs down the port side of the boat. The vent doesn't have anything to do with the engine exhaust, only the skipper's.
My answer to your problem with rebedding the stansions was to cut four holes in the liner so I could access the underside of the stantions and then to cover the holes up with some varnished teak. I cut some backing plates to sandwich the deck in between the stantion and the backing plate and make the stantion sturdier. All of this might have been a waste of time since you might have to take the teak toerail off some time in the future and you would end up using a 7/16 socket on a long extension with a swivel. In my case I found that the most difficult stantion was the one above the ice box, if you cut a hole here be sure to get it as close to the stantion as possible.
