You've described it perfectly. It is essentially a C#7 when played two frets up from the B7 position and Willie sometimes hits the E bass and definitely avoids the 4th string 3rd fret, giving a minor feel, i.e. C#m7, the relative minor, and a good substitution chord for E. Mostly he bounces around between the 5th, 3rd and 1st strings. The open 2nd string works OK in the C#7 as it is the dominant 7th, but should be avoided at the middle position, C7. As I said, Willie mostly stays on the 5,3, and 1 strings, but in transcribing Broke Down Engine, I found he played the turnaround differently in every verse, but the left hand was always the same, as you described it.
