In a "Standard" 12 Bar blues, fills wil not alter the bar structure. The singing tends to fall in bars 1-2, 5-6, and 9-10, and the fills will most often fall in bars 3-4, 7-8, and 11-12, in response to the vocal phrases. Modern ensemble styles of blues like Chicago Blues stick to this format pretty strictly, for the most part, but many Country Blues players varied phrase lengths, and thus ended up with forms other than 12 bars in length or 12 bars in length but with some measures that had other than four beats per measure. You can find structures like this in the music of such players as Blind Lemon Jefferson, Furry Lewis, Charlie Patton, Robert Wilkins and many more.
