The attacks I have seen look like skin twitching on the minor end up to muscle twitching. I haven't been around for anything worse. The owner keeps Karo syrup at the barn for when an attack starts. I believe she gives something like 50cc every 20 minutes until the attack ends. It has to be karo syrup - not sugar or any other type of sweetener.
The vet recommended hand walking the horse during an attack.
There's quite a bit of info out on the web at university sites (including UCD) on managing the condition.
