Good for you on getting the hay tested! The link is below. Just use test #601 - it's only $26 and that's all you need.
ALL hay needs to supplemented in some way in order to balance the minerals and add anything that is outright deficient, such as selenium. You can test separately for selenium, but it's an extra $35 and since northern CA soils are selenium deficient anyway, I wouldn't bother spending the extra money.
If you want to post the results when they come in (it usually only takes a week - 10 days from the time you mail your sample) I'll run over it for you here on the board so that everyone can learn.
