I created a spreadsheet where I track my horse's vaccinations, and use that to remind me what he needs and when he's due. Spring / Fall vaccinations, dental, etc, etc. I've been using it for a number of years now & found it works well.
I based it off of this vaccination schedule, published by UC Davis (meshed with my own experience & talking with my vet):
