Am so sorry to read that you have ongoing problems. You mention that you have tried 'the fodmap diet', but I wonder if this was with assistance from an expert dietitian or just on your own.
As I understand it, Fodmaps is not a 'diet'. It is a way of identifying which food(s) may, in effect, be fermenting in your gut. Through elimination of the 'offending' foods for a period of time, you can then slowly reintroduce the same foods without encountering the same problems.
Perhaps Google 'Shepherd et al', and Fodmaps at Bart's, St. Thomas's and King's College.
