His body composition at that size greatly suggests steroid use. All I can say is trust me when I say I know what it looks like. I also know what 250lbs looks like and Mac was way above that.
As Tom alluded to, there are many, many types of steroids that do many different things. You can take certain types that will not put on muscle mass, but greatly increase your endurance and recovery. So not everyone that gets caught has to be a mountain of muscle.
