According to Hugh Roberts (The Queen's Diamonds, 2012), this brooch was made with 6 diamond drops and 197 brilliants provided by Queen Elizabeth and a further 52 brilliants supplied by Cartier. Roberts mentions that "The Queen has selected the brooch for wear on a small number of formal occasions", but as far as I know, the only time this brooch had been publicly seen on Queen Elizabeth II was for the State banquet for President Jacob Zuma of South Africa at Buckingham Palace in March 2010:
It was worn regularly (though not really on a frequent basis) by Queen(-Mother) Elizabeth in the 1940s/1950s/1960s, but almost never later:
