You asked a very interesting question. I am no expert but I would say that, today, even though she was awarded legitimately according to the statutes of the time, HM shouldn’t wear the insignia and/or the sash for three reasons:
1) The current statutes of the Portuguese Orders determinate that the sash and insignia can only be worn by the Portuguese Head of State in office;
2) In 1993 (27th April) the Grand Collar of the Order of the Tower and of the Sword was awarded to Queen Elizabeth II. Apparently only 3 foreign heads of State have this grade: Queen Elizabeth II, King D.Juan Carlos of Spain and King D.Filipe VI of Spain;
3) I believe that HM is quite rigorous in what concerns the correct use of orders and decorations.
Nevertheless, we know that HM Queen Elizabeth sometimes make her own rules. See what happened in her “recent” visit to France: she was not supposed to have worn a tiara but she did.
