    I always thought of this piece as a convertible bracelet adapted as tiara by adding a central piece, just as you say, most likely a brooch.
    I can't see no visible frame, it seems to be wraped around her high chignon, and the center motif stands some centimeters below the level of the side pieces.
    But it does look very nice, and it is perfect match for the hair style. In fact I find it would be very dificult to wear this jewel with any diferent hairdo, unless a frame was set to the base.
