Edit: I am happy to run again the story of the V&A if newer members would like to read it, with pics of course.

Bob, as I understand the history, what we know today as family orders began with Edward VII and have continued in that tradition, very like the George IV.

In earlier times the monarch gave a jewelled item as a personal "thankyou" I suppose, more or less.

Victoria and Albert started with a miniature given to a daughter, and that evolved into the Royal Order of Victoria and Albert - which was not confined to family.

From Royal Orders, Hugo Vickers, 1994
" Queen Victoria created her own Order, the Order of Victoria and Albert, which was founded on 10 February 1862, and given to Princesses of the Blood Royal and other ladies favoured by the Queen. She wore a decoration with a badge on which the Prince Consort's head was cut in front ot hers. All the others were the other way round. "

    : Thanks Nellie, I've never seen QV's
: miniatures, although I have seen the William
: IV buckles. Now that you mention them, I
: remember that they were used instead of
    : family orders.
    :
: I guess QV did not want to be associated
: with George IV, so would not want to wear
    : his order, and William's buckles don't
: easily lend themselves to wearing. Makes
    : sense.
