Thank you, Beth1! I am also puzzled by the way how royal ladies wear their larger brooches or stomachers. In the Netherlands, the large sapphire stomacher has been worn by Queen Beatrix and by Queen Maxima once each, and it looks af if both dresses are made of a thick fabric (I would say velvet for Queen Beatrix's dress): 1981: Queen Beatrix's State Visit to Luxembourg (with a breast star pinned on the dress, at heart's level): 1 February 2014: celebration of (former) Queen Beatrix's reign: But Queen Juliana wore in 1980 an even bigger stomacher (from the Mellerio ruby parure), and yet, the fabric of her dress seems to be a rather thin and supple fabric. I do not know if Queen Juliana wore a corset or a reinforced structure underneath her dress (note that she also wears two order stars, one pinned at heart's level, the other one near the hip): Same remark in 1984 when Queen Beatrix wore, for French President François Mitterrand's State Visit, the large bow-shaped sapphire corsage brooch. Her dress looks to be made of silk, I do not know if the brooch is pinned on something more solid under the dress, to help supporting the weight of this large brooch. The breast star of the French order of the Legion of Honour is pinned on the sash, at waist level:
