Lady Susan Hussey was raised to the status of Dame Grand Cross of the Royal Victorian Order (GCVO) in 2013 hence the sash and (larger) breast star in the 2015 picture (upper).
She is also wearing medals (from left to right) for
    Royal Victorian Order
    QEII Silver Jubilee Medal
    QEII Golden Jubilee Medal
    QEII Diamond Jubilee Medal

In 2004 (the lower picture) Lady Hussey had been awarded her Dame Commander of the Royal Victorian Order – hence the (smaller) breast star and no sash. The sash is only for GCVOs. She is also wearing medals (from left to right) for
    Royal Victorian Order
    QEII Silver Jubilee Medal
    QEII Golden Jubilee Medal

Here is a picture of Lady Susan Hussey attending in 1966 (on the right). Then she only wore the diamond monogram "E" on a pink bow of a Lady-in-Waiting.

With her are the Duchess of Grafton, current Mistress of the Robes (left) and the Duchess of Devonshire , then Mistress of the Robes (centre)
