Initially, I thought that, perhaps, the distance between the two tiers had been increased to accommodate the articulated diamond in the centre and that, perhaps, might explain the addition of extra leaves when the jewel was converted to a tiara. (I haven't seen it worn as a necklace)
But, apart from the differences you observed, I think that the angle of the larger diamonds sprouting from the leaves is different. Perhaps I am mistaken. As we all know, my eye is not brilliant.
