I'm sorry, I don't completely agree with this. In California a document cannot be notarized if there are any blanks. At least put N/A. The Signature Affidavit is also good if the person refuses to sign exactly as name is printed. They can print the name in this space as it appears on documents and then sign the way they say their signature is suppose to be. However; I would call the NNA Hotline and ask them or refer to your State Laws.
