The majority of smokers do NOT end up getting lung cancer, and IF there is actually a spot or shadow on your lung (which is unclear at this point, from what you say) that still doesn't mean smoking caused it, as TB exposure, pollution, and exposure to various organic solvents and some particulates can cause similar X-ray results.
In the unlikely event it does turn out to be something that's smoking-related, I would probably choose to tell the whole truth to the doc to help her or him correctly diagnose the problem. Whether or not you do tell the whole truth, however, if the issue is cancer, any treatment would probably still be the same because it will depend on the type of cancer as determined by cell biopsy. As for your wife, wouldn't it be very easy to blame a smoking-related problem that turns up on your PAST smoking?
