chemo on consent form

I have been reading a few posts lately regarding waxing and chemo. I have never had a client on/after chemo ask me for waxing, but I am now educating myself just in case.

Should I include this question in the consent form? It seems pretty important and if they just neglect to tell me I could have a major problem.
