I will say that ever since I got the bt-micro skin spatula extractions have never been easier (even on the nose) that thing wiggles blackheads out like nobody's business and painless to boot!
Before that I always did a peel of some kind,lots of steam and if the person already had broken caps used anything other then finger cots on those areas otherwise patience and equal pressure with an extractor works well and if you can use lancets to get things started that always helps
