The Southcentral Foundation (SCF) Case Management Support is responsible for performing the coordination and management of administrative duties for a Primary Care Provider (PCP) team as well as the wider Integrated Care Team (ICT). Working together with the core medical team the CMS is a primary contact between the customer-owner and ICT ensuring effective and efficient schedule coordination; verbal and written communication; information research and transfer; and system coordination.
Behavioral Health Aide - McGrath
The Southcentral Foundation (SCF) Behavioral Health Aide (BHA) provides preventive, early intervention, case management, aftercare and follow-up services to village residents impacted by a variety of behavioral health issues that may include addiction/substance abuse and mental health problems. Services are provided under the direction of the Clinical Supervisor and within a defined scope of practice as defined by the Behavioral Health Aide Program. This position ranges from a Trainee to a BHA III.

The Southcentral Foundation (SCF) Community Health Aide is responsible for providing acute, chronic, emergency, and preventive medical health care to Village residents under the direction of the medical doctors in Anchorage and according to the CHA/P Manual. The CHA Trainee (CHA-T) level is a training classification for those who do not yet qualify as CHA I. The CHA I, II, III, IV and Practitioner are expected to operate within the guidelines of the CHA/P Manual and their level of training and the scope of practice consistent with their level of training. CHA II, III, IV and Practitioner are also expected to act within the limits of their Standing Orders (SO) when applicable.

Southcentral Foundation is seeking a dedicated Community Health Aide Level 4 or Practitioner to join the rural health team on a rotational schedule. The rotational schedule includes 4 weeks working with 4 weeks off or 6 weeks working with 4 weeks off.

Summary of Job Responsibilities: The Southcentral Foundation (SCF) Maintenance Technician is responsible for providing maintenance for grounds and common areas of all SCF-owned buildings including basic/simple repair of equipment, maintenance of facilities, and mechanical systems that do not require a trades license, installation or reconfiguration of work stations and shelving/wall hangings and landscaping. This position has three (3) levels designed to provide progressively more responsible and independent work experiences. Progression between job levels is based on the demonstrated ability to successfully handle more progressively responsible assignments.
