Im not fully convinced the Spencer tiara was offered and if it was why that was not the piece Meghan wore for her wedding. Of course HM would have understood the special significance of a jewel worn by Diana. She did not stop William from using Diana's engagementring for Catherine why would she stop a loan for a Spencer-tiara for Meghan?
I find the entire story a bit dubious. The only way i can think of it happening is if Harry and Meghan did not mention to HM that she was planning to wear the Spencer-tiara (she would be the first to my knowledge who wasn't the bride of a Spencer or a female line Spencer herself).
Maybe Meghan and Harry wanted to see if HM would loan her something even more impressive. Meghan might have thought that the Queen was talking about her personal tiara's that were publicly worn by HM.
How they got that idea i don't know because HM loaned the Cartier Halo tiara to a future Princess of Wales and Queen of the United Kingdom so Meghan was not about to get something that much bigger as the bride of a younger son.
We don't really know what pieces Queen Elizabeth still had in her vaults and that were not seen for decades. Meaning we can only speculate about what pieces Meghan did get to try on. Meghan may have been hoping for the Vladimir tiara in the emerald version but that was never an option.

The Emerald tiara mentioned by Mr Spill the Beans in his 'memoire' can only be the Greville emerald tiara his cousin Eugenie wore on her wedding. Not really all emerald but what does he know. Not much about anything we now know. He didn't even know how the Letters Patent of 1917 applied to his own offspring. No one can seriously believe QEII at any given time would have considered offering the Vladimir Tiara with the Cambridge emeralds as a loan to Magpie for her wedding. Come on!

According to Spare the tiaras were chosen by the Queen. My guess is the two mentioned were the Greville Emerald worn by Princess Eugenie and the Aquamarine tiara was the one worn the Duchess of Edinburgh.
It says that the offer was made quite late on after the dress had been designed to match the Spencer tiara. Which means that the offer to Princess Eugenie could also have been made late on. It doesn’t make much sense that a tiara would be offered to Meghan and then refused because they forgot it had links to Russia. The ceremony wasn’t connected to Russia. The royal family regularly wear jewellery with Russian links, except in Russia where steps are taken to be diplomatic. It makes sense to me that if Eugenie was offered five tiaras they would probably consist of the four tiaras declined by Meghan (because the Queen thought they were appropriate) plus one added, possibly the Mary Fringe, assuming that wasn’t one of the four. We don’t know if the Queen thought that the fringe was only for blood princesses. The fact it was only worn by the blood princesses doesn’t mean it had to be.
“ Next came the question of a tiara. My aunts asked if Meg would like to wear my mother’s. We were both touched. Meg then spent hours and hours with her dress designer, getting the veil to match the tiara, giving it a similar scalloped edge. Shortly before the wedding, however, Granny reached out. She offered us access to her collection of tiaras. She even invited us to Buckingham Palace to try them on. Do come over, I remember her saying. Extraordinary morning. We walked into Granny’s private dressing room, right next to her bedroom, a space I’d never been in. Along with Granny was a jewellery expert, an eminent historian who knew the lineage of each stone in the royal collection. Also present was Granny’s dresser and confidante, Angela. Five tiaras were arrayed on a table, and Granny directed Meg to try on each one before a full-length mirror. I stood behind, watching. One was all emeralds. One was aquamarines. Each was more dazzlingly stunning than the last. Each took my breath. I wasn’t the only one. Granny said to Meg quite tenderly: Tiaras suit you.”
