It's a composite photo, given away by the very awkward position of the baby on Wilhelmina's lap. You can see the faint swirls around the "tiara" and her head that show it was touched up and added to.

In my opinion its a 'photoshop' from the ornate pearl tiara.

The "baby" doesn't even look real, it looks like it was drawn in by an artist. The arms are out of proportion and the face is definitely not the face of a real infant. You can see the swirls above the tiara.

Yes this can only be meant to be the Ornate Pearl Tiara. There are similar pictures of Queen Wilhelmina in this awkward pose.
