A culinary tour of Sicily

My friend and I have been dreaming about Sicily for a long time and want to go on a gastronomic tour — travel around small towns, taste local dishes, and find those trattorias that are "untouched by tourists." But I don't know if it's realistic to travel by public transport between villages and wineries. They say that the routes there are chaotic and buses may not arrive on time. So I'm thinking: maybe we should plan to rent a car right away, but I've never done that in Italy. Won't it be difficult with parking, traffic fines, or insurance?
