J-F, I agree that the main decoration is made from fabric (my mother used to call it 'posamentrie', a kind of textile art).
Just my impression: enlarging the picture to 200% the base of the center choker also looks textile to me. Reminds me somewhat of the kokoshnik velvet bases with stones sewn onto them.
