It wasn't obvious that the tiara of the Provinces would go from queen to queen. It was Leopold III who decided to hand this piece to his daughter-in-law Fabiola. Queen Fabiola was not obliged to hand it to her sister-in-law only by doing so did she establish something of a tradition making that particular jewel one to be worn by the Queen of the Belgians.
