Actually, I think the central diamond and sapphire piece of that choker (in the Getty Image photo posted by LittleMo) comes from either one of the pair of earrings or from the ring (shown as part of the box jewels in the Getty Images photo) because of its sunray design.
I agree with LittleMo's thought that several sapphires (surrounded by diamonds) in the watch could have been worn by Princess Diana as earrings. There are also some sapphires on the top of the jewel box itself.
In the Getty Image picture of Diana wearing the choker in LittleMo's post, the choker is made up of a diamond band, a sapphire/diamond center and some series of blue stone beads.
In Frank's post, a picture of Diana with another choker includes a velcro on which a diamond band with the flat-looking sapphire/diamond center is laid. Both chokers looked different in design and appearance except maybe the diamond band in each picture.
