I agree with you, Maria-Olivia. Queen Mathilde's electric blue dress was very elegant and stylish, with these long open sleeves (I am less convinced by the hat...), but a brooch would have done no harm.
Queen Fabiola's "waterfall" brooch could have been an option. Queen Mathilde has worn this brooch for the weddings of the Prince of Monaco, the heir-in-second to the British throne and the Hereditary Grand-Duke of Luxembourg, so I think it would not have been "over the top" for the wedding of the Hereditary Archduke of the ducal throne of Este-Modena...
Anyway, Queen Mathilde could also have worn another diamond brooch, in the shape of a heraldic rose. It is smaller than the waterfall brooch, but still sizeable, very nice and regal anyway.
Queen Mathilde has worn it at least three times since 2008, so I suppose it is hers, and not a borrowed jewel:
