I think that is in fact a tiara (or head bandeau) worn with the spanish mantilla. As I said, it is a well known fact how the spanish first lady adored jewels. As some first ladies in those days, she wore tiaras at state visits; also on her elder grandaughters weddings. She is wearing it here at the weding of her eldest grandaughter Maria del Carmen with the Duke of Cadiz. Here's another pic:

The pearls on the necklace and earrings are really huge and ostentatious (that was the purpose I think). As to the head ornament/tiara...let's say I would not buy it even if I had the Money (to put it very politely). Perhaps it was one of the "gifts" .
