Somehehere it is said as Queen Sofia did not wear tiaras at State visits to Republics Queen Letizia is following her example. Pity that she does it not her own way as she does in other things but i guess she doe not like to wear tiaras and one wears one when necessary.

And I wonder as to why was last year's state dinner in honour of the Dutch royal couple in Germany a tiara event (Queen Máxima wore the smaller version of the House Diamonds tiara for the first time, but no sash, only a small ribbon pin with an order) and the same did not happen in the Spanish visit. It was also a black tie event but the Dutch queen wore a tiara, so my guess is that it was a personal criteria of both queens.
So, bravo to Queen Máxima and Queen Mathilde for keeping these black tie events as tiara events, as apparently Queen Letizia only wears tiaras when visiting other monarchies.
